Welcome to the YWCA Contra Costa/Sacramento Childcare Program.
Welcome to the YWCA! Before you come in for your enrollment appointment, you will have to complete the Childcare Registration Package you can access below.
Please give yourself 45 minutes to 1 hour to complete. You will have an option to save your application and go back to it when you are ready. Please have the following documentation ready to upload at the end of the application.
- Proof of income for the last 60 days (employment pay stubs/child support/SSI/etc/...for Parent A & Parent B if applicable)
- Birth certificates (all children in the home under age 18)
- Current Proof of address ( with parent(s) name and address)
- Current immunization records (printout acceptable)
The Childcare Registration Package contains the following sections:
- CHILD’S PREADMISSION HEALTH HISTORY—PARENT’S REPORT
- IDENTIFICATION AND EMERGENCY INFORMATION CHILD CARE CENTERS/FAMILY CHILD CARE HOMES
- Emergency and Identification Information
- CONSENT FOR EMERGENCY MEDICAL TREATMENT - Child Care Centers Or Family Child Care Homes
- MEDICAL STATEMENT TO REQUEST SPECIAL MEALS AND/OR ACCOMMODATIONS
- PERSONAL RIGHTS
- CHILD CARE CENTER NOTIFICATION OF PARENTS’ RIGHTS
- NEEDS ASSESSMENT
- PHOTOGRAPH RELEASE FORM
- VIDEO CAMERA ACKNOWLEDGEMENT
- ACKNOWLEDGEMENT OF RECEIPT
